If you are thinking of becoming a patient at Fallon Clinic please:


We can help you with selecting a Primary Care Physician, talk you through the insurance process, help you schedule a new patient appointment, and answer any questions you may have.

Patient Visit Preperation

We suggest arriving for your visit 15 minutes prior to your scheduled appointment to allow for check-in time and the filling out of any needed forms. Please keep the following in mind when visiting Fallon Clinic:
  • Remember to bring your insurance card with you
  • Please be prepared to pay your co-payment at the time of check-in
  • Credit Cards (MasterCard® & VISA®) as well as cash or check are accepted for payment
  • Bring a list of the medications you are taking, or bring your prescription bottles with you
  • If you are a new Fallon Clinic patient, please contact your previous provider 2 to 4 weeks prior to your appointment and request that a copy of your records be sent to your new Primary Care Physician (PCP).

If for any reason you cannot keep your scheduled appointment, please contact your physician’s office at Fallon Clinic at least 24 hours in advance so that we may accommodate other patients needing an appointment. We will be a happy to assist you in rescheduling.

What You Can Expect

Fallon Clinic believes in a team approach to clinical care. Your physician leads a team of Medical Assistants, Licensed Practical Nurses and Registered Nurses to provide medical care for you. Fallon Clinic also utilizes highly trained Nurse Practitioners and Physician Assistants to assist in your evaluation and treatment. Each of these clinicians works under the direct supervision of your attending physician.

Your physician is your partner for health and wellness. He or she performs your medical evaluation and with your participation sets up and implements a treatment or intervention plan. Throughout your course of treatment, your physician coordinates the input and actions of all the members of the care team to provide the best possible outcome for you.

Your nurse will play an essential role in caring for you. You can expect your nurse to do everything from simple procedures such as giving an injection to providing medical advice and follow-up care on the phone. Feel free to consult with your nurse whenever you have a question or concern regarding your health.

Nurse Practitioners and Physician Assistants are highly trained medical specialists that perform some of the same tasks that doctors do. They work in direct collaboration with the attending physician and will often play a large role in your care experience. Fallon Clinic Nurse Practitioners and Physician Assistants handle everything from prescribing medicine to seeing patients. At Fallon Clinic, we all work together to provide the best care available for you and your family.
